Important details for Physical Night:- Date: Thursday, April 14th- Cost: $25 (will accept credit card, cash and checks) by appointment only using the link to Signup Genius – https:\/\/www.signupgenius.com\/go\/10c0c49adae28a0fec25-gcphysical1<\/a><\/p>\n\n\n\n

– Paperwork needed:      –  Parental Consent (completed and signed)
     –  IHSAA Pre Participation Physical form (history portion filled out before appointment)
– If your student-athlete has tested positive for COVID 19 in the last 6 months then your doctor will do a more comprehensive evaluation and a “Return to Play” clearance will be given.  If your student-athlete has already had the cardiac exam and has a “Return to Play” clearance, please bring a copy with you.<\/p>\n\n\n\n

– Please show up 10 minutes before your scheduled appointment.<\/p>\n\n\n\n
